REINTEGRATION SCATTER SITE CASE MANAGER
Sign on bonus of $3,000 for qualified candidates when hired. Sign on bonus will be paid in increments of $750 at 3 months, 6 months, 12 months, and 18 months.
DISTINGUISHING FEATURES OF THE RSS CASE MANAGER POSITION: The work involves responsibility for counseling and offering guidance to a variety of clients in a clinical or residential setting. Duties are performed as a part of an interdisciplinary team. Work is performed under direct supervision of the Program Coordinator and Clinical Director with leeway allowed for the exercise of independent judgment in individual cases. Work is reviewed through clinical supervision and review of records. The Case Manager performs related work as required.
TYPICAL WORK ACTIVITIES OF THE RSS CASE MANAGER: (Illustrative only)
Participate as a member of the interdisciplinary team in program planning, evaluation of client needs, and treatment planning and implementation of direct services;
Determines appropriateness of client housing;
Maintains weekly phone contact with program participants;
Participates with multi-disciplinary team in analyzing individual client problems, and developing short/long range strategies and interventions;
Serves as liaison with aftercare and other service care providers including Dept. of Social Services (DSS),with the overall goal of coordination of client services;
Counsel clients on basic addiction and recovery issues, living skills, the use of social and recreational outlets, and educational and vocational opportunities;
Prepare and maintain required reports, case records, progress reports and similar documentation.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES OF A RSS CASE MANAGER: 1) Working knowledge of the principles, practices and techniques used in group and individual counseling. 2) Working knowledge of human behavior, socio-economic problems and development. 3) Ability to recognize psychiatric symptomatology and to participate in formulating effective treatment plans. 4) Ability to establish, maintain and terminate a counseling relationship. 5)Ability to establish and maintain effective working relationships with others. 6) Ability to communicate effectively, both orally and in writing. 7) Ability to maintain records and prepare reports. 8) Displays good judgment and tact.
MINIMUM QUALIFICATIONS OF A RSS CASE MANAGER:
A. Graduation from a regionally accredited or New York State registered college with a Bachelor’s degree; or
B. Graduation from a regionally accredited or New York State registered college with an Associate’s degree, preferably in a human service field, and two (2) years of experience in direct human services provision, preferably within the substance use disorder field
A supervised internship in a chemical dependency setting is equivalent to one year of employment experience.
SPECIAL REQUIREMENTS OF A RSS CASE MANAGER: Must possess a valid driver's license with less than three (3) points and a good driving record.
