Key Responsibilities:
- Client Assessment and Treatment Planning:
- Conduct initial and ongoing assessments to gather information on clients' needs, strengths, and goals.
- Collaborate with clients to develop individualized treatment plans that address their substance abuse challenges, mental health concerns, and recovery objectives.
- Establish realistic goals and actions to promote and maintain sobriety, well-being, and a healthy lifestyle.
- Case Management and Coordination:
- Serve as the primary point of contact for clients, providing ongoing support, education, and resources throughout their recovery process.
- Collaborate with external service providers, such as healthcare professionals, social services, and community agencies, to coordinate access to necessary resources and ensure a continuum of care.
- Coordinate and advocate for clients in accessing mental health services, medical care, housing, employment, education, legal support, and other needed services.
