Discharge Coordinator jobs in Binghamton, NY

Schuyler Hospital is looking for a dedicated Full Time Discharge Coordinator in our Primary Care Clinic. This candidate will be responsible for coordinating care for patients following provider visits including booking follow-up appointments, coordinating and tracking referrals, and obtaining prior authorizations for services. Reviews and prepares patient charts for pending appointments, acquires records as needed, and performs various office functions.

Requirements:

Education

• Graduate of accredited NYS Medical Assistant or Certified Nurse Assistant preferred

Experience

• Relevant experience in a patient care setting required; knowledge of medical terminology required. Possess good oral and written communication skills, basic computer skills necessary

Licensure

• None

Physical Requirements

• Repetitive use of hands and fingers (ex: use of computer keyboard),
• May require lifting and carrying light loads (up to 20 pounds), including boxes, supplies, sitting, walking, or standing for long periods of time
• Must be able to physically perform CPR/BLS measures
• Must be able to physically operate the equipment used for the job
