Discharge Coordinator directs daily operation of record processing to ensure that discharge records are properly received, organized, and forwarded to the appropriate physician for completion. Audits medical records to guarantee compliance with institution standards, procedures and policies. Being a Discharge Coordinator may require a bachelor's degree in area of specialty. We are seeking a dedicated and detail-oriented Case Manager/Discharge Planner to provide comprehensive case management, screening, and assessment, and works with a multi-disciplinary team to coordinate services for clients in the jail system. Provides direct assistance, psycho-education, relapse prevention skills, and advocacy to assist clients in obtaining appropriate resources to improve client outcomes and quality of life after being released from jail.
Minimum Requirements: Bachelor’s Degree preferred or Associate’s degree in Human Services or related field with three years related experience and/or training; or equivalent combination of education and experience preferred. LSW or LPC required. Three years’ experience in the mental health field required; Experience working with individuals experiencing SUD and COD concerns preferred. Valid Ohio Driver’s license, use of personal car, and annual training standards as required by the Center.
