Established in 1950, Maunalani Nursing and Rehabilitation Center is a 100-bed skilled nursing facility located on Wilhelmina Rise in Kaimuki. We take great pride in caring for our Kupuna. The Discharge Coordinator holds a key position in the organization, and as such, must fully embrace and promote our organization’s culture of pride in ownership. Responsibilities and Duties:

Essential Job Function includes, but not limited to providing assistance to evaluate and assess residents' psychosocial needs and concerns, complete the federally required MDS, work with interdisciplinary team members, ensure document for residents' progress and response to care are provided, follow requirements in survey guidance manual strictly by performing consistent and frequent interviews with residents, families and staff. Oversee and coordinate the discharge process from discharge date to discuss with charge nurse any nursing concerns, document discharge plans in medical records, arrange transportation with family or Transportation Company, ensures all residents have a POLST document available on file and any other assigned duties as needed.

Education and Experience: Bachelor’s in social services or any related Human Services degree with Healthcare experience preferred.

Requirements: 2 step TB clearance, and Criminal background check and drug screen clearance are also required.

Job Type: Normal work hours vary and must be available to work weekends.
