    Position summary: Admission/Discharge Nurse

    Role Qualifications: Admission nurse is a nurse licensed with the State of Tennessee Board of Nursing that has relevant working experience in the Admission and discharge process. Their main goal is to provide high quality patient care services and increase their satisfaction by facilitating and managing all aspects of admissions and discharge.

    Position responsibilities:

    • Work together with Hospital Case Managers to receive and approve Skilled and LTC referrals.
    • Work in conjunction with MDS to update and approve financial information for potential admissions
    • Work in conjunction with MDS to update daily census
    • Perform administrative duties including keeping admission records for each patient.
    • Handle the patient admission aspect including choosing room placement for potential admission, entering MD orders into EMR, work with case managers to ensure all medical equipment is available.
    • Prepare, complete and register admission paperwork
    • Enter Physician Orders into EMR and work with Pharmacy to ensure orders have been received and are shipped
    • Manage and solve admission problems with family, case managers and nursing staff.
    • Work in conjunction with nursing staff and Social Services to have Admission paperwork signed by resident, POA or Responsible Party.
    • Design, develop and carry out nursing care plans in conjunction with MDS
    • Interview patients about their medical history.
    • Establish and maintain positive relationships with case managers, patients and their families
    • Collaborate with nursing staff and attending doctors in establishing patient admission, discharge or transfer information communicating with nursing staff in a timely manner
    • Establish a discharge care plan meeting and give Discharge packet information
    • Ensure compliance with health care admission policies and regulations
    • Work with Social service department in discharging process
    • Work with nursing department to ensure discharge education is performed when applicable
    • Superuser for computerized chart to include training for nursing staff in EMR, making changes to EMP and updating/formulating assessments in EMR
    • Active member or organization committees and Eden Employee training
    • On call for Admission process
