Discharge Coordinator directs daily operation of record processing to ensure that discharge records are properly received, organized, and forwarded to the appropriate physician for completion. Audits medical records to guarantee compliance with institution standards, procedures and policies. Being a Discharge Coordinator may require a bachelor's degree in area of specialty. We are hiring an Admissions/Discharge RN.
M-F RN weekend call rotation.
Current GA RN License required.
1year LTC experience required, but 3-5 years experience preferred.
The Admissions/Discharge RN coordinates admissions & discharges working along side Admissions/Marketing, Social Services, and DON to ensure all requirements for admission and discharge are met.
The appropriate candidate will utilize analytical and assessment skills to determine residents required level of care and communicate information to appropriate departments.
Strong communication & documentation skills are a must.
An understanding of LTC rules and regulations will be required.
Communicating with residents, families, physicians, & the IDT to ensure the best outcomes for admission and discharge.
Be liaison between facility and physicians during the admission and discharge process for medication reconciliation, assessments and resident needs. Participating in facility management and care plan meetings.
Participates in ongoing education to enhance knowledge of current & new regulations.
Conduct audits to ensure compliance & survey readiness.
Take on-call hours on weekends and holidays as assigned
