New Horizons is seeking an organized DISCHARGE PLANNER, with the ability to multi-task and work within' a fast pace environment.

 As a Discharge Planner your responsibilities will include:

• Provides discharge-planning activities for clients to include assessment of strengths and needs, services planning services brokering and client advocacy
• Participates with team members in daily program activities
• Coordinates discharge planning and appropriate aftercare services for assigned clients
• Prepares written documentation for each service conducted

Requirements

New Horizons will need you to have the following qualifications:
