Pre-employment drug testing, FBI criminal background check and education/degree verification required.Chesterfield County Mental Health Support Services is seeking a part-time Senior Clinician to serve as the Discharge Assistance Program (DAP) Coordinator to support the community reintegration of individuals discharging from psychiatric state hospitals. Functions include participation in treatment team meetings; coordination with State Hospital Liaisons to support the development and execution of discharge plans; development of Discharge Assistance Program plans; administrative oversight and ongoing monitoring of DAP plans; participation in regional DAP meetings; and establishing and maintaining relationships with State Hospital Liaisons, state hospital staff, community providers, other CSBs, Region 4 and DBHDS stakeholders. Perform other work as required.
