Audits medical records to guarantee compliance with institution standards, procedures and policies. Being a Discharge Coordinator may require a bachelor's degree in area of specialty. As a Center for Independent Living (CIL), we are dedicated to the Independent Living Philosophy and are seeking team members who are passionate about working alongside people with disabilities to achieve their goals.
As an Independent Living Coordinator, you will guide individual participants in the identification of their independent living goals and provide resources, training, coaching, and other supports to ensure that they meet their stated goals. You will also assist with creating and building community relationships, organizing workshops, classes, or activities, and inspiring the independence of all program participants, with a focus on providing CDCS support planning, social security applications and appeals, and independent living skills assessment services.
These are Rochester- and Winona-based, in-office positions that are full time and benefit eligible. These positions provide support to counties outside of Olmsted and Winona, and some travel is required.
