The unit utilizes a multidisciplinary approach of RNs, LPNs, CNAs, Care Management, Acute Therapies and Physicians to ensure that patients receive a holistic approach to optimizing their care as they transition out of the hospital. Nursing staff aim to provide care to a wide variety of diagnoses, across multiple medical specialties. They will continue to provide wound care, pain management and patient teaching as appropriate to maintain or improve baseline functional status of the patient.
