A Healthcare Nursing Facility in Zephyrhills Florida is seeking a Discharge Coordinator. Nurses encouraged to apply. As the Discharge Coordinator, you will play a crucial role in ensuring smooth and efficient transitions for our patients from facility to home or other care settings. Working closely with multidisciplinary teams, you will coordinate discharge plans, educate patients and families, and facilitate access to post-discharge resources and support services.

Key Responsibilities:

Develop and implement individualized discharge plans in collaboration with patients, families, and healthcare providers.

Coordinate with healthcare team members to ensure timely and appropriate discharge arrangements, including scheduling follow-up appointments and arranging for home health services.

Provide patient and family education regarding post-discharge care instructions, medications, and community resources.

Collaborate with case managers, social workers, and other healthcare professionals to address barriers to discharge and optimize patient outcomes.

Maintain accurate documentation of discharge plans, patient education, and coordination activities.

Qualifications:

Psychology or social work background preferred.

Or an Active Nursing License.

Experience in inpatient/geriatric setting.

Strong communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and interdisciplinary teams.

Excellent organizational skills and attention to detail.

Knowledge of healthcare resources and community support services.
