To be a successful Acute PRTF Discharge Planner you must become able to do the following always:
Responsible for overseeing the discharge planning from an acute setting or Psychiatric Residential Treatment Facility (PRTF) program by assisting the youth’s successful transition back into the community.
Liaison with other stakeholders involved in the client’s treatment i.e., therapist, clinical director, Community Mental Health Center (CMHC) representative, Permanency Services Team, family members, the client, and next placement providers.
Schedule a discharge staffing 10 days prior to discharge for youth in an acute PRTF placement with identified stakeholders
Ensure services are in place and assist with referrals to support youth with extreme or unique needs.
Follow and monitor all discharged youth for 6 months post discharge to ensure a stable and successful transition has occurred and avoid failure to place instances.
Ongoing two-way communication with internal placement services, Permanency Services Teams, and Residential PRTF provider as well as community providers.
Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
To qualify for this job, you need to have:
Bachelor’s degree in social services related field required.
License – BSRB license preferred.
Work Experience – Two years’ experience working in child welfare, residential or related field.
Strong organization and time management skills.
Demonstrate knowledge and skills necessary to provide age and level of care-appropriate assessment and care relative to the client being served inthe acute care setting.
21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
Maintain company-required training.
Ability to travel throughout the state.
