DISCHARGE COORDINATOR/PHARMACY TECHNICIAN
Full Time; M-F; 8am to 5pm

Job Summary
:

The discharge coordinator serves an integral role by acting as a liaison between the pharmacy team and an inpatient facility servicing the mental and behavioral healthcare community. These duties include but are not limited
to
building strong relationships with clinic staff (providers, nursing staff, case managers, and
consumers
), assisting with coordination of delivery to the unit, insurance barriers, and the full services of the Janus RX pharmacy. This is a great opportunity to utilize the skillset of a pharmacy technician in a non- traditional environment. A key function will be promoting Janus RX services to onboard and retain consumers and patients. A successful discharge coordinator will have strong communication skills and be able to perform detailed work.

Job Duties
:

• Enroll consumers in our services and aids in continuity of care by coordinating prescription refills between providers’ offices and subsequent delivery of medication to the patient


• Work directly with providers’ offices and/or facilities as part of an interdisciplinary team-based approach to medication management


• Advocates for patient health by navigating common barriers such as prior authorizations, patient assistant programs, sync fills, or delivery


• Coordinates the delivery of medication to the unit prior to the patient discharging from the inpatient facility

Minimum Employment Requirement(s)

• Possess and maintain a valid professional license/certification/registration issued by the State Board of Pharmacy


• High school diploma or GED


• This position has access to and knowledge of extremely sensitive, private, and materials and requires the highest standard of confidentiality

Preferred Qualification(s):

• Pharmacy Technician Certification Board (PTCB) certification One year of experience as a pharmacy technician OR


• Two years of healthcare experience prior to hire
