    Role Overview:

    In this vital role, you will be the cornerstone of our patients’ successful transition from hospital to home. Discharge Planner, you will ensure that each patient receives personalized and comprehensive planning to meet their unique needs during this critical period.

    Key Responsibilities:

    • Conduct thorough assessments of patients’ social, emotional, and physical needs to craft tailored discharge plans.
    • Collaborate with a diverse team of healthcare professionals to ensure continuity of care and the integration of needed services.
    • Arrange and coordinate essential services such as home health care, medical equipment, and community resources to support patients post-discharge.
    • Act as an advocate for patient rights, ensuring that their preferences and cultural needs are respected.
    • Educate patients and families on the discharge process, available support services, and community programs.
    • Provide emotional support and counseling to assist patients and families during the transition period.
    • Manage documentation requirements efficiently, including discharge summaries and insurance authorizations.
    • Work collaboratively with insurers and case management teams to secure timely service approvals and coverage.
    • Monitor patient progress after discharge, adjusting care plans as necessary to ensure optimal outcomes.
    • Uphold stringent confidentiality and accuracy in patient record management in line with regulatory standards.

    Skills and Qualifications:

    • Bachelor’s degree in Social Work or a relevant field preferred.
    • Current licensure or certification in social work preferred.
    • Experience in case management, discharge planning, or social services within a healthcare environment.
    • Comprehensive knowledge of medical terminology, healthcare systems, and community resources.
    • Strong problem-solving skills and the ability to identify and address patient needs effectively.
    • Excellent communication skills to interact effectively with patients, families, and medical staff.
    • Proficiency in navigating complex healthcare environments and managing time effectively under pressure.
    • A compassionate approach, offering support to patients and families during sensitive transitions.
    • Skillful in using electronic health records and Microsoft Office software.
