Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. We are looking a qualified candidate who can help us continue to achieve our mission of delivering the highest quality non-medical home care services to seniors in Wyoming.

    We currently have an amazing opportunity available in Casper for an experienced business professional to join our care team as our Client Care Manager. The Client Care Manager is responsible for a variety of duties as they oversee the coordination of our care services between clients and caregivers in the market. In addition, this person will be responsible for developing relationships of trust by networking with leaders in the Casper senior/health care community.

    The Applicant must be compassionate, empathetic, and love working with seniors. Customer relations experience and the desire to thrive within a high growth industry are also a must. This would be an outstanding position for someone who comes from the hospitality field.

    This is a Full-Time position and pay will be based upon experience.
