Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Assists the Case Management Team with facilitating the safe and timely transition of patients from Skilled Nursing Facility to lower levels of care and all related out patient follow up needs.
- Understands both the importance and the processes necessary to ensure patient-specific continuity of care during their transition to a lower level of care.
- Interviews patients and/or families to assist the SNF Case Manager Associate with the identification of potential barriers to discharge, including but not limited to, financial, emotional, inadequate support systems, and/or other patient-specific barriers.
- Will communicate relevant patient-specific findings or concerns to the SNF Case Manager Associate and assist the SNF Case Manager Associate in addressing applicable issues, which might impede a safe and timely transition to a lower level of care.
- Identifies patients whom are appropriate for and would benefit from Priority Home and Custodial Program (PHCP), Disease Management and/or other high-risk programs offered by Desert Oasis Healthcare. Upon direction by the Physician or SNF Case Manager Associate, will gather the applicable supporting data necessary and generate referrals to these programs.
- Assists SNF Case Manager Associate, family, and SNF Provider to facilitate a safe effective discharge plan that meets the needs of each patient. Such duties include but are not limited to faxing information for scheduling outpatient specialty consultations, coordinating Home Health requests, ordering Durable Medical Equipment (DME).
- In the absence of the SNF Case Manager Associate, will act as their Designee and assist the SNF Provider with facilitating timely and safe discharge and with any relevant continuity of care needs.
- Coordinates all authorization requests for services needed which are not available in SNF setting (i.e,Speciality Provider Consults, Radiology, Transportation, etc.).
- Attend weekly SNF ICT meetings and provides constructive input towards patient goals along with a timely & appropriate discharge plan of care.
- Ensures that NOMNC/denial letters are initiated timely and according to Health Plan and regulatory agencies specifics
- Maintains accurate and complete SNF Case Management review files and ensures these are scanned into the Document Manager for future reference
- Emails a SNF PIP Disposition report on every DOHC SNF member’s discharge to the appropriate DOHC email groups.
- Ensures a POLST document is discussed and/or accurately completed with every DOHC SNF admission
- Emails the SNF Medical Records packet to the appropriate DOHC email group with every DOHC SNF member discharge.
- Responsible to complete all due 5 Star Quality Measures for Skilled and Non-Skilled DOHC members that are able to be completed in the SNF setting.
- Assists the SNF SS Rep to initiate Medi-Cal Applications on applicable DOHC SNF admits.
- Other duties as assigned within scope of responsibility.
- High school diploma or equivalent required. A minimum of 2-years college education preferred.
- Minimum 2 years previous experience in the medical field, preferably in the acute care hospital. Experience should include direct contact with patients who have disabilities or other significant acute or chronic conditions. Previous experience with discharge planning which included direct patient/family interaction preferred.
- Familiarity with medical terminology.
- Strong computer ability.
- Must possess excellent telephone, and people skills, with an ability to interface with DOHC staff and outpatient providers in a pleasant and professional manner.
