Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. Summary
Responsible for ensuring clients leave treatment with a solid aftercare plan, safe housing and further needed treatment. Assist in meeting overall census and budget goals.
Essential Duties and Responsibilities
• Provides quality direct and indirect client care to individuals with substance abuse or chemical dependency problems.
• Assess clients continuing care/referral needs.
• Establishes rapport with clients.
• Obtains the necessary information and releases from client, family, referral sources and other appropriate resources.
• Makes referrals to Mental Health and Drug & Alcohol facilities for continuing care.
• Sets up Halfway House interviews and placements.
• Works closely with counselors and members of multi-disciplinary team.
• Attends and participates in program, clinical and administrative meetings.
• Actively supports the facility’s ongoing referral program.
• Participates in mandatory orientation and training activities.
• Other duties as assigned.

Licensure, Education, & Experience
Southeast and Silver Line
• High School diploma required. Bachelor’s Degree in a human service related field preferred.

Work Environment
The noise level in the work environment is usually moderately quiet & indoors. This position will be scheduled based on operational need & will be required to provide own transportation. Incumbent may be exposed to virus, disease, and/or infection from clients in the work environment. Incumbent may be exposed to traumatic situations (i.e. psychiatric).
