Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Inpatient Coordinator / Discharge Planner

  • The Inpatient Coordinator is responsible for supporting the process that manages all hospital and skilled nursing admissions, concurrent review, and discharge planning. The goal of the Inpatient Care Coordinator is to effectively support the in-patient management process by assuring the right information is provided timely to the RMG administrative management team, hospitalists’ and care managers.

     

    Essential Duties and Responsibilities include the following: 

    1. Anticipate our customer needs: Understand the wants and needs of customers, listen for cues and identify how to respond and what level of intervention they need.
    2. Refrains from discussing personal business in front of the customer or speak a foreign language different from the customer’s language.
    3. Greets patients, family members, and physicians in a positive manner by introducing self, speaking clearly and with confidence.
    4. Returns phone calls to customers and follows up with requests.
    5. Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner.
    6. Demonstrates honesty and integrity in everyday activities.
    7. Protects privacy for both patients and employees; ensures all personal health information is kept confidential i.e., (sensitive papers, charts, and reports are not in view of the public).
    8. Recognizes when an error has been made and immediately reports to appropriate manager.
    9. Functions as liaison between care managers, patients, physicians, and other healthcare providers.
    10. Communicates appropriately and clearly to management, co-workers, and physicians.
    11. Communicates using age appropriate language.
    12. Consults other departments as appropriate to provide for an interdisciplinary approach to the patient’s needs.
    13. Maintains quality monitors as assigned by Supervisor.
    14. All other duties as assigned by manager.
    15. Participates in orientation, instruction/training of new personnel.
    16. Participates in the efficient, effective, and responsible use of resources such as supplies and equipment.

     

    Critical Elements: 

    1. Census will be verified every morning with regards to in house status and a review will be requested.
    2. Census reports should be printed daily before every rounds meeting.
    3. Authorizations for Home Health/DME/Hospice/Follow up appointments will be done coordinating with Care Managers.
    4. Request for authorizations will be given via fax/return phone calls.
    5. Monitor campus fax server and process accordingly.
    6. Communicate to Medical Director or designee, immediately any admissions to non-contracted hospitals.
    7. Provide face sheet to Medical Director or designee, immediately upon receipt and eligibility check.
    8. All new admissions are to be checked for Out of Area status.
    9. Confirmation of inpatient status of all inpatients will be obtained by 10 am for reporting at morning case rounds.
    10. Assist Inpatient Care Managers with planning for discharge by generating authorizations and scheduling appointments for services needed within 7 days of discharge.
    11. Communicate with High Risk/outpatient department(s) any services identified upon discharge that is necessary beyond 7 days after discharge.
    12. Support the safe, continuous delivery of medically necessary care and supportive ancillary services in the inpatient and transition to home care setting.
    13. Execute all post discharge needs (ie: PCP, Specialist appts, diagnostic studies, etc).
    14. Identify network needs and report to management for potential contracting opportunities.
    15. Review case statuses to ensure proper documentation and case closure. Work with assigned Care Managers to complete all documentation.

     

     

    The pay range for this position at commencement of employment is expected to be between $18 and $20 hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

     

    The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
