The Social Services Designee/ Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident/guest discharge planning.

The Discharge Planner will assist the social service department with:

• Meeting the resident on or shortly after the admission and following until discharge to the community.  May attend the 72 hour care plan meeting to attain needed information. 
• Confirm contact information on face sheet.
• Ordering, or recommending resources to acquire, durable medical equipment as needed/recommended by therapy for mobility or nursing to attain optimal health status and ordered by the practitioner. 
• Arranging home health care for after discharge with the resident/guest preferred provider, as possible. 
• Ordering and coordinating  IV home infusion services, wound vacs, trach supplies, tube feeding supplies, vents and vent supplies, if necessary
• Coordinating nursing education with proposed caregiver 
• Overall coordination of the discharge
• Attending care conferences
• Attending Medicare Meetings
• Any item related to the discharge process for residents who will be going home
• Ensuring completion  of the Post Discharge Plan of Care and Summary
• Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.

QUALIFICATIONS
