Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.     Position Summary:

    • Effectively utilizes hospital resources in the development of care, discharge planning and Utilization Review.
    • Utilization Management performs prospective, admissions, observation and concurrent review according to standard policies and procedure.
    • Utilization Management reviews and analyze processes designed to ensure appropriate utilization of medication, blood products, operative and invasive procedures, and use of resource intensive diagnostic testing and treatment.
    • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
    • Assists with LTC, SNF, Rehabilitation, Behavioral/Mental Health, Home Care, DME, transportation and other placement planning needs.
    • Assists with interactions with CPS, APS, Adoptions and end of life planning and completion of Advance Directives.
    • Act as a liaison with third-party payers and government programs to access special services for patients.

    Qualifications

    Minimum Qualifications:

    • ADN or BSN required.
    • Current licensure applicable to professional affiliation required.
    • Certification by the National Association of Healthcare Professionals or InterQual is desirable.
    • Current New Mexico licensure as a Registered Nurse.
    • Certification as CCM or ACM desired.
