Discharge Planner -Hickory Behavioral Network
Direct Report to Director of Clinical Services
Primary Purpose and Function:
The Discharge Planner will provide quality case management and coordinate care for discharging patients. This includes working directly with family, friends, collaborators, and caretakers of patients (when permitted by appropriate release of information) as well as outpatient providers, programs, and agencies to assist patients in the development of an appropriate discharge plan.
Minimum Qualifications:
Bachelor’s degree is human services field preferred
Nursing License, Social Worker License, or 1 years coordinating utilization review and discharge planning activities
Computer proficiency, including Excel, Word, Publisher, and EMR use required
Impeccable ethics, integrity, and outstanding interpersonal and communications skills
Positive demeanor and clear understanding of professional boundaries with patients with sound clinical judgement
Ability to follow directives explicitly, adhere to facility policies, provide patient-driven customer service, and exercise time management skills
Knowledge of MAT or substance use disorders- helpful
Schedule flexibility, holiday, and weekend rotation may be required
Primary Responsibilities:
Participates as a member of a multi-disciplinary treatment team by collaboratively engaging with all disciplines regarding concerns or barriers to discharge plan.
Responsible for documenting each engagement with the patient and anyone involved in the patient’s care.
Timely and complete medical documentation.
Manages assigned discharging patients with appropriate discharge plans in place and communicate discharge plans to external agencies.
Participates in all Performance Improvement and Quality measures.
Upholds familiarity with State and Federal laws and regulations.
Adherence to facility policy and procedures, State & Federal law, and Joint Commission accreditation regulations and standards.
Pay Band: Full Time, $18-25/hr
