Overview:
We are seeking skilled and compassionate Licensed Practical Nurse (LPN), Registered Nurse
(RN) as Discharge Planner to join our healthcare team. As an LPN or RN, you will play a vital
role in providing quality patient care and assisting in the overall coordination of medical services
to the date when they will be discharged from the facility. Specifics to Discharge Planning
includes assessment, prognosis and social support system/person information are included in the
initial assessment process

Duties:
1.Demonstrates professional responsibility in the role of Discharge Planner
2. Complies with personnel policies and hospital safety policies
3. Maintains confidentiality when interacting with patients, families, personnel and the public
4. Maintains compliance with State/Federal Guidelines and standards
5. Performs duties as prescribed by the Discharge Plan of the facility and as directed by the
Nurse supervisor or by the Medical Doctor.
6. Conforms to all requirements of Medicare, Medicaid and other insurance policies
7. Reports any problems to the Nurse supervisor in a timely manner
8. Identifying of specific needs of residents, and social service intervention or referral while in
the facility.
9.Awareness of services available to patients and their families are an important part of
Discharge Planning.
10.Determining patient needs on a daily basis base on their diagnosis
Qualifications:
-Valid LPN or RN license
-BLS
-Experience using Sigma would be great
