Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. Summary:
As a member of the PEMHS treatment team, you will complete individualized assessments, treatment planning, case management, and discharge planning for consumers during their stay. Compliance with all documentation requirements is mandatory.
Benefits:
Education: Bachelor’s Degree in Social Work or Human Services field
Experience: One year of experience as mental health social worker (preferred) or one year of other relevant work experience.
Physical Requirements: Must be able to sit, twist, bend, and stand. Must be able to communicate both orally and in writing.
Special Requirements: Exposure to consumers; treatment program conditions, high turnover in consumer census with specific paperwork on each consumer.
