Discharge Planner - Home Care jobs in Spokane, WA

Coordinates and tracks utilization management authorizations from third-party payers, including pre-certification and concurrent and retrospective authorizations. Gathers, coordinates and facilitates medical record information required for authorizations and appeals management for Medicare/Medicaid/Commercial payers. Assists in preparing, maintaining and disseminating utilization review reports and communications. Utilizes communication, organizational, and problem-solving skills to ensure successful implementation of the plans and interventions to facilitate utilization review and discharge planning processes. Assists with initiatives set by the organization as needed. Familiar with a variety of the field’s concepts, practices, and procedures. Relies on experience and judgment to plan and accomplish goals. Provides and documents discharge planning services for all patients admitted to our hospital. Coordinates and documents patient referrals, transfers, and discharges. Performs other related duties as assigned.

• Ability to lift up to 25 pounds.
• Frequent stooping, bending, reaching, lifting when filing and accessing records.
• Read charts, dials, equipment settings, literature and other printed and handwritten materials.
• Communicate with patients, physicians, families, and co-workers in person and on the telephone.
• Work with equipment and manipulate equipment settings, computer keyboard, etc.
Maintains confidentiality of all hospital and patient information at all times. Follows HIPAA regulations and policies.

Minimum Qualifications:

• Graduate of a nationally accredited Nursing program required.
• State of Idaho Registered Nurse licensure pending as documented by temporary licensure or current RN licensure from a state participating in the “multistate privilege to practice” compact with Idaho prior to start date.
• Minimum of 1-year experience with Electronic Healthcare Record systems
• Knowledge of medical and clinical terminology, DNV, Medicare rules, DRG’s and billing
• Demonstrated proficiency in computer applications related to position duties with a variety of software
• Demonstrated proficiency in written, verbal and phone skills
