Discharge Planner - Home Care jobs in Torrance, CA
Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. - *REQUIREMENTS:**
To qualify for this position, the successful candidate must:
- Social Service Designee Certificate
- Have 1-2 years of long-term care experience
- Be able to communicate effectively in writing and verbally
- Be in good physical and mental health
- Be able to perform physical duties such as lifting weights of up to 40 pounds
- Be able to follow written and oral instructions
- Have training, knowledge, and experience to allow for general supervision of responsibilities
- *REPORTING:**
This position reports to the Administrator.
- *JOB DUTIES:**
The successful candidate will be responsible for the following:
- Participating in the resident admission process to ensure implementation of the plan of care
- Participating in interdisciplinary team meetings as assigned
- Providing ongoing communication with family members, as indicated
- Ensuring appropriate education training is conducted before discharge
- Monitoring medication regimen to ensure appropriate utilization
- Serving as a role model for interdisciplinary team
- Promoting a positive, calm, and effective work environment
- Monitoring for noncompliance trends and making recommendations
- Serving on committees as assigned by the Administrator
- Performing any other duties as assigned by the Administrator.
