Job Summary
Responsible for assisting in the overall coordination and delivery of social work services as it relates to Memorial Hospital and Health Care Center. Demonstrates knowledge of developmental needs of patient population specific to departments within the Division. (Patient population is defined in each department's Scope of Practice.) Individual will provide social work assistance in accordance with the mission and core values of Memorial Hospital and Health Care Center.
Qualifications
Education:
Bachelor in Social Work or equivalent field required. Masters preferred. Member in good standing of the National Association of Social Workers preferred.
Licensure:
Licensed Clinical Social Worker or Licensed Social Worker in the state of Indiana or a plan to obtain licensure is preferred.
Training:
Approximately two to three years social work experience in a health setting.
Experience:
Two to three years social work experience in a health care setting.
Job Knowledge, Skills, and Abilities:
Demonstrates an understanding of social work in a health care setting. Able to use independent judgment and work alone as frequent independent decisions are required and can have impact on policy, cost, and/or patient care. Excellent communication and written skills as well as a sound and healthy emotional well-being. Position requires a high degree of concentration, analytical and/or creative thinking over extended period of time. Ability to interact effectively with all levels of the organization. Ability to communicate departmental duties and/or roles, community resources, and legal information to others in a clear, understandable manner. Computer skills are needed for electronic documentation purposes.
Performance Requirements
Assists in the delivery of social work services as it relates to Memorial Hospital and Health Care Center. Duties may vary day to day in the sense of working with diverse populations. May work with, but not limited to, the following populations: acute care, emergency care, and home care. Primary job duties will be discharge planning for hospitalized patients and providing social work services primarily in the acute, emergency and home care setting. Provides at least an initial assessment to all new patients, unless circumstances warrant not doing so. Works to meet the discharge planning requirements for all patients determined to be in need of those services. Must understand and demonstrate an ability to provide discharge planning and works together with Case Management and Physicians in respect to DRGs and utilization review.
Participates in overall quality improvement plan and departmental meetings/updates. Keeps abreast of departmental Policies and Procedures, as well as Hospital wide Policies and Procedures. Completes all necessary documentation for patient in a timely manner, complying with all Federal and State mandates. Keeps current of pertinent State and Federal guidelines and law.
Keeps current with available community resources and how to access them.
Assignments given by the Social Work Supervisor and/or Clinical Manager/Director of Behavioral Health and Social Work Services. Duties may vary day to day in the sense of working with diverse populations. May work with, but not limited to, the following populations: acute care and home care. This is a PRN role to allow for additional coverage as determined by departmental leadership. Weekends and holidays may be required.
