Where do you fit?

To be a successful Acute Hosp/PRTF Admission & Discharge Planner you must become able to do the following always:  

1. Responsible for assisting with admissions to Acute Psychiatric Hospitals and Psychiatric Residential Treatment Facility (PRTF) programs. 

1. Responsible for overseeing the discharge planning from an Acute setting or Psychiatric Residential Treatment Facility (PRTF) program by assisting the youth’s successful transition back into the community. 

1. Liaison with other stakeholders involved in the client’s treatment i.e., therapist, clinical director, Community Mental Health Center (CMHC) representative, Permanency Services Team, family members, the client, and next placement providers. 

1. Schedule a discharge staffing 10 days prior to discharge for youth in an acute PRTF placement with identified stakeholders 

1. Ensure services are in place and assist with referrals to support youth with extreme or unique needs.  

1. Follow and monitor all admissions. 

1.  Follow and monitor all discharged youth for 6 months post discharge to ensure a stable and successful transition has occurred and avoid failure to place instances. 

1. Ongoing two-way communication with internal placement services, Permanency Services Teams, and Residential PRTF provider as well as community providers. 

1. Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
