Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. Aspire has health centers in five Central Indiana counties serving Hoosiers of all ages and walks of life.
Position Summary
The Infectious Disease Prevention Case Manager will provide behavioral risk assessments, care planning, referrals, re-evaluations, and discharge planning to individuals participating in the Infectious Disease Prevention programs. Additionally this position will provide HIV/HCV/STI prevention education to the community, as well as HIV/HCV/STI antibody testing in community and office settings. Staff are responsible for the implementation, evaluation, and reporting as required by funders. Staff will seek and maintain productive partnerships with key stakeholders. This position serves a multi-county region and the hours for this position are dependent on funding.
