Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager.     Job Summary and Qualifications

    The Case Management Educator enhances the professional practice and the provision of quality patient care by providing evidence-based quality educational programs and activities that promote professional competency for the case management department personnel.


    What you will do in this role:

    • Participates in the development, coordination, managing, facilitating, conducting, and evaluating of onboarding and orientation programs for case management personnel.
    • Provides orientation/onboarding programs to promote employee retention.
    • Conducts orientation in a manner that facilitates a positive learning environment.
    • Incorporates organizational values/culture into the orientation process.
    • Promotes cost effectiveness through implementation of efficient orientation/onboarding practices.
    • Participates in the development, coordination, and management of facilitating, conducting, and evaluating competency.
    • Collaborates with the Case Management Director to review outcomes data to evaluate and refine competency management activities.

    What qualifications you will need:

    • Required Experience: 3 years of experience in an acute care setting
    • Preferred Experience: 2 years of experience in an acute care case management setting
    • Required Education: Bachelors Degree in Social Work and or Nursing
    • Preferred Education: Masters Degree in Social Work and or Nursing
    • Required License: Currently licensed as a registered health professional in the state(s) of practice and/or has an active compact license, in accordance with law and regulation.
    • Required Certification: BLS
