Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. Territory: The San Jose market is Santa Clara Co from Gilroy(South) to Milpitas on the East and North up to Palo Alto/Stanford/Los Altos. Every attempt is to keep Case Managers as centrally located within their particular territory - we are in a crunch in this market so a candidate that is flexible will be most helpful.Hospice Nurse Responsibilities:
- Overseeing the care of patients who are nearing the end of their lives.
- Including, but not limited to; scheduling patient visits in accordance with needs, physician collaboration, orders management, medication management
- Supervises LPN's / LVN's / Hospice Aides per protocol
- Work with a team of medical professionals to ensure that these individuals receive high-quality palliative care, including physical, emotional, and spiritual support.
Qualifications:
- Associates degree, preferred
- Registered Nurse license required
- Minimum one year of experience
- Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.
- Willingness to serve on-call
- Must have reliable transportation & valid driver's license
