Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager.     Adult Care Management, Inc. (ACMI), a long-standing Colorado-based nonprofit case management organization serving the counties of Boulder, Broomfield, Gilpin, and Clear Creek, has opening positions on our case management team for full-time Case Managers.

    For over thirty (30) years ACMI has been contracted with the state of Colorado to provide case management to children and adults receiving services from CO Long Term Care Medicaid waivers in our area.

    ACMI Case Managers visit with clients in a variety of settings (e.g. community, hospitals, and nursing facilities) with a variety of people (e.g., persons with physical disabilities, brain injuries, major mental illness, and the frail elderly) for a variety of programs and community services. In addition to other work tasks and responsibilities (see below), the state of CO requires all case managers to complete in person visits for new and existing members.

    Essential Tasks of the Case Manager Position

    • Intake Case Managers complete level of care assessments and service plans for consumers newly applying for Medicaid waivers, LTC Nursing Facilities, and state plan programs.
    • All Case Managers make Medicaid program eligibility determinations with a state prescribed assessment tool.
    • Ongoing Case Managers develop and document service plans based on the service needs of the consumer, identified by the assessment.
    • All Case Managers adopt and practice person-centered thinking and assessment practices to ensure an overall personal and individualized approach to working with individuals.
    • Ongoing Case Managers authorize services in accordance with Medicaid program rules and ACMI procedures.
    • All Case Managers educate potential and current clients about community resources as well as Medicaid services.
    • Other general case management duties include: providing thorough and timely documentation of all work activities, providing thorough and timely processing of application paperwork and requisite forms, faxing, filing, and clerical duties as needed, commuting to various client settings in the community. communicating clearly and effectively both in-person and on telephone, lifting and carrying up to 50-lbs of client-related files, using intuition, experience, and theory to analyze complex situations and needs, being self-reflective and responding to difficult situations with logic and thoughtfulness, writing clearly and effectively, and preparing written and verbal summaries of complex client needs.

    Requisite Cognitive Abilities

    1. Ability to track multiple deadlines and tasks efficiently; 2. Ability to identify and resolve problems efficiently and effectively; 3. Ability to gather and analyze both detailed and contextual data; 4. Ability to use intuition, experience and theory to analyze complex situations and needs; 5. Ability to translate system rules, agency policies and work processes to clients and peers as needed; 6. Above average organizational skills; 7. Ability to understand and follow agency procedures and policies for implementation of program rules and requirements.

    Requisite Relational Abilities

    1. Ability to respect and be sensitive to complex disability needs and personalities; 2. Ability to be self-reflective and respond to difficult situations with logic and thoughtfulness; 3. Ability to listen carefully; 4. Ability to discern the questions that need to be asked; 5. Ability to contribute to team dynamics; 6. Ability to coach others and address conflicts directly with a focus on solutions. 7. Ability to represent the agency to the public, providers and contractors.

    Requisite Technical Abilities

    1. Writes clearly and effectively; 2. Uses Microsoft Office products effectively and efficiently; 3. Ability to run effective internet searches for resources; 4. Ability to use and understand databases; 5. Ability to prepare written and verbal summaries of complex situations, including interpersonal conflicts and competing system demands.

    Mandatory Requirements

    Minimum of a Bachelors degree strongly preferred, a behavioral science degree preferred (e.g., human services, psychology, social work, nursing), but other degrees considered with relevant experience. Some higher education such as an Associate's degree along with relevant life experience or previous employment in human services work may substitute for Bachelor degree.

    Case Management experience preferred, but not required.

    Valid Driver's License.

    Reliable transportation for commuting to member homes, hospitals, nursing facilities, and the office, when needed.

    Passing both a general background check and a CO Adult Protection Services registry check are required.

    Salary Range $39,500* to $55,000** - non-exempt hourly
