Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. The Case Manager facilitates and monitors the provision of cost-effective healthcare services that meets the patient’s needs in a timely manner, in the most clinically appropriate setting. Collaborates with the patient/family, physician, healthcare team, payer representatives, utilization review and post-acute service providers to assess, plan, implement, coordinate, monitor and evaluate options and services to promote optimal patient outcomes. Ensures compliance with quality patient care and regulatory compliance. The Case Manager provides care management, care coordination and discharge planning to ensure that the patient progresses through the continuum of care. Serves as a liaison for patients/families in the healthcare system and the community to coordinate self-management support and appropriate post-acute referrals of chronic diseases for assigned populations. Utilizes team approach to promote and maintain plan of care, problem solving skills and nursing process to achieve optimal patient outcomes. Has responsibility to direct shift activities for Occupational Therapy consistent with departmental and corporate policies and procedures including flexing staff based on operational needs and enter time in time keeping system for designated employees. May provide input only for evaluations, hiring, coaching, and skills development, and discipline. This position has access to confidential patient, system and financial information. This position makes decisions independently and/or in collaboration with others and may have patient contact.
EDUCATION
Bachelor's Degree with concentration in Physical Therapy, Occupational Therapy, or Speech Therapy
REQUIRED CERTIFICATION
Current Ohio License as a Physical, Occupational, or Speech Therapist
