Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager.     The Hospital Care Coordinator responsibilities include:

    • Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning
    • Possesses a clear understanding of the available resources to promote quality cost effective outcomes
    • Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served
    • Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning
    • Ability to adjust to changing priorities in a fast paced environment


    Required Certifications and Licensures:

    • Required RN (Registered Nurse)
    • Preferred ACM (Case Management)
    • Preferred CCM (Case Manager)


    Education:

    • Required Associate's of Nursing or
    • Preferred Bachelor's of Nursing


    Experience:

    • Required 2 years in Acute Care
    • Required 2 years Registered Nurse

    The Case Management Assistant responsibilities include:

    • Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care.
    • Responsible for assisting with distributing regulatory documents to identified patients.
    • Educating patients about the appeal process related to their discharge.
    • Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service.
    • Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives.
