Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. As one of our CARES-giver, you will help expand our impact in the community by providing access to high quality, compassionate health care.

    Here's a sneak peek at what you'll do:

    • Work with the HHM's providers and healthcare team members so that every Ryan White patient receives the support they require and has access to available resources
    • Provide education and assistance to individuals eligible for the Ryan White program.
    • Meet with patients and complete Health Risk/Needs Assessments, develop Service/Action/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications.
    • Establish Health Risk/Needs Assessments, Action/Service/Care Plans, referrals, and support; your work will directly contribute to success in meeting the deliverables of our program
    • Work with a diverse patient population in the heart of Dallas
    • Share your expertise and best practices to help launch our new service line

    What you need to succeed

    To become one of our CARES-givers, you will have excellent patient engagement, strong communication skills and an eye for continuous improvement. You will also have the following:

    • Associate's or Bachelor's in nursing from an accredited school
    • Current licensure as a Registered Nurse or Licensed Vocational Nurse from Texas Board of Nurse Examiners
    • Minimum of 3 years experience in a Infectious Disease
    • Current BLS certification
    • Prior Ryan White Care Act Program experience
    • Experience serving multicultural and multilingual patient populations
    • Experience with eClinicalWorks or similar and MS Office Suite skills
    • Bilingual in Spanish, Pashto, Burmese, Rohingya or other languages a plus
    • Experience in community health centers and/or non-profits is a plus.
    • Have a servant heart with the desire to make a positive impact
