Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge.     Site supervisors oversee the day-to-day operations of their site. Including but not limited to: scheduling, supplies, supervision.

    This position reports to the Area Medical Services Manager.
    Responsibilities
    Patient Care

    • Rendering care to injured/ill workers as necessary in accordance with organizational standing orders.
    • Determining if first aid is adequate or if transport to a physician, ER, or trauma center is needed. Coordinate transport as needed (911).
    • Coordinate return to work of injured workers.
    • Following up as required with patients and physicians.
    • Provide patient education pertaining to disease progression and relevant therapies.
    • Participate with contracted disability management vendors to assure client progression.
    • Advocate with other case managers and other service providers on behalf of workers.
    • Provide regular reports on patient health status.
    • Collect urine drug, saliva, and breath alcohol tests following chain of custody.

    Clinic Operations

    • Maintaining log on employees seeking treatment and treatment records.
    • Maintaining all safety records to include accident reports.
    • Maintaining inventory of supplies and ordering supplies as needed.
    • Ensuring that all shifts are scheduled in accordance with the client's needs while adhering to the overtime policy.
    • Maintain a clean, organized, and inviting patient environment.

    Teamwork and Cooperation

    • Maintaining relationships with medical providers, trade partners, and clients

    • Act as liaison between employees, employer, insurance company, and medical service providers.
    • Working with supervisors in the area of patient medical needs and filing supervisor’s accident reports.
    • Advising and/or assisting supervisors in conducting investigation of accidents.

    Qualifications

    • Current Registered Nurse (RN) License, Licensed Practical Nurse (LPN) License, in good standing within state of practice.
    • Current CPR card.
    • Computer literacy and proficiency in electronic health records software, email, word processing, spreadsheets, and internet based applications.
    • Excellent verbal, written, interpersonal, and organizational skills.
    • Strong Management skills
    • Flexible hours required, including some evenings and weekends.
    • Ability to pass criminal background check and drug screen.
