Disease Management Case Manager jobs in Evansville, IN
Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.Job Responsibilities- Interviews patients, assesses priorities, and writes reports while managing complex caseload.
- Completes comprehensive assessments of patients.
- Assists patients/families with referrals to community agencies and provides education and resource information.
- Works in collaboration with the interdisciplinary team to ensure quality support of care and safety of discharge.
- Collaborates with leadership on complex discharge planning issues and delays in service.
- Provides timely and concise documentation in the medical record, the assessment, concerns, needs, and discharge plan.
- Keeps plans up to date as patient progresses toward discharge.
- Supervises and/or trains other new employees, students, or interns as needed.
Qualifications- Associate's degree or higher in Social Work required upon hire
- No experience required
- LSW - Licensed Social Worker in the State of Kentucky required upon hire OR
- LCSW - Licensed Clinical Social Worker in the State of Kentucky required upon hire OR
- CSW - Certified Social Worker in the State of Kentucky required upon hire
