  • Job Summary: We are seeking dedicated Case Managers to join our team for the Texas Children and Pregnant Women Case Management Program C.A.R.E.S. As a Case Manager, you will be responsible for providing comprehensive case management services to children and pregnant women in Texas, ensuring they receive access to necessary healthcare, social services, and support. This is a contract position with the opportunity to make a meaningful impact on the lives of vulnerable populations.

    Responsibilities:

    • Conduct initial assessments to determine the needs of children and pregnant women enrolled in the program.
    • Develop individualized care plans in collaboration with clients, outlining goals, objectives, and interventions.
    • Coordinate access to healthcare services, including medical, dental, and behavioral health care, as well as prenatal care for pregnant women.
    • Advocate for clients and ensure they receive timely and appropriate services from community resources, including but not limited to housing, transportation, food assistance, and childcare.
    • Monitor client progress and reassess needs regularly, adjusting care plans as necessary to promote positive outcomes.
    • Provide navigation opportunities to local education and support to clients on topics such as healthy lifestyle choices, prenatal care, parenting skills, and accessing available resources.
    • Maintain accurate and up-to-date documentation of client interactions, assessments, care plans, and progress notes in accordance with program requirements.
    • Collaborate with interdisciplinary teams, including healthcare providers, social workers, educators, and community organizations, to ensure coordinated and holistic care for clients.
    • Participate in program meetings, trainings, and quality improvement initiatives as required.
    • Adhere to all program policies, procedures, and ethical standards, maintaining client confidentiality and privacy at all times.

    Qualifications:

    • LBSW-IPR, LMSW-IPR, LCSW, RN (bachelor or advanced degree in Nursing) required; Master's degree preferred.
    • At least two years of experience in case management, social services, or healthcare coordination, preferably working with children, pregnant women, or underserved populations.
    • Knowledge of Texas healthcare and social service systems, including Medicaid and CHIP, preferred.
    • Strong interpersonal skills with the ability to build rapport and establish trust with clients from diverse backgrounds.
    • Excellent communication skills, both verbal and written, with the ability to effectively advocate for clients and collaborate with interdisciplinary teams.
    • Ability to work independently, prioritize tasks, and manage time effectively in a fast-paced environment.
    • Proficiency in computer skills, including Microsoft Office and electronic health record systems.
    • Bilingual proficiency in English and Spanish is preferred. (Incentive if Bilingual)
    • Trainings are not paid and this position starts after you are contracted and credentialed with at least two manager care organizations. This position is paid based on the event and is not paid by the hour.
