Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will be responsible for providing support and guidance to individuals and families in need. This role requires strong interpersonal skills, the ability to assess client needs, and the coordination of services to ensure the best possible outcomes.

Duties:
- Conduct comprehensive assessments of client needs and develop individualized care plans
- Collaborate with clients to set goals and monitor progress towards achieving them
- Provide counseling and emotional support to clients
- Coordinate services and referrals to ensure clients have access to necessary resources
- Advocate for clients' rights and navigate complex systems on their behalf
- Maintain accurate and up-to-date case records
- Collaborate with a multidisciplinary team to ensure coordinated care
- Stay informed about community resources and stay up-to-date on best practices in case management

Requirements:
- Bachelor's degree in social work, psychology, or a related field (Master's degree preferred)
- Proven experience in case management or a related field
- Strong knowledge of social services and community resources
- Excellent communication and interpersonal skills
- Ability to assess client needs and develop appropriate care plans
- Strong organizational skills and attention to detail
- Ability to work independently and as part of a team
- Proficient in computer applications such as Microsoft Office
