Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. The Case Management Assistant (CMA) will provide support and assistance to the MSW Social Workers and Case Managers in the completion of discharge planning responsibilities including, but not limited to, ordering DME, arranging transportation and coordinating with nursing units. The CMA documents in the patient’s medical record timely and per regulatory requirements. Documentation may include entries regarding reimbursement data as directed. The CMA cross-trains and assists the Intake Planner as needed. The CMA will be flexible in accepting job assignments while displaying good organizational and prioritization skills. This position requires independent decision making and functioning. Provides conscientious commitment to assigned work, work schedule, honesty, teamwork, and assisting others to bring about positive outcomes. Weekend and holiday rotation is scheduled based on the needs of the department and hospital. The Case Management Assistant will work from referrals from Care Coordination staff. The CMA will respond to resource requests for social work or case management assistance from physician offices and other community agencies.
