Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. Palmetto Case Management is looking for a field nurse case manager with a rehabilitation and/or emergency trauma background. Workers Compensation experience is a plus. Palmetto nurse case managers follow the injured person's care and treatment plans from start to finish, facilitate medically necessary services and address long-term needs, coordinate the different levels of care necessary, bridge communication between all parties, including the injured person, family, medical providers, insurance carriers and the employer and provide ongoing support to the injured person and their family.
Home office plus travel between worksites and clients. Position requires driving of personal vehicle for completion of tasks.
