Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. Manager - Family Case Management

  • Job Duties:

     

    1. Oversees and directs the development of policies and procedures for assigned programs to ensure quality care and compliance with funder/payer and accreditation body.  Supervises, is accessible to and provides direction to staff.  Provides leadership and guidance to staff through appropriate utilization and implementation of the policies and procedures.  Ensures programs are operating according to funding, licensing and accreditation guidelines.  Monitors documentation compliance with internal procedures, contract requirements, payer requirements, accrediting standards, and Florida Statutes and Administrative Rules.
    2. Supervises Family Case Management staff to ensure they are complying with all required reporting and tasks as assigned for their role.  
    3. Develops, constantly improves, implements, and monitors systems to ensure the smooth operation of program managed, ultimately resulting in excellent customer service, the timely and accurate completion of clinical documentation, outcome data, and service event entry.
    1. Lead day to day operations of all aspects of the family case managers.
    2. Provides outreach and presentations internally and in the community for programs supervised.  
    3. Remain current in the general knowledge of psychotropic medication, side effects, and DSM-V

             diagnoses, and recognize when medical services are appropriate.

    1. Respond to customer (internal & external) complaints and concerns. 
    1. Participate in or lead Suncoast Center committees, group supervision and meetings as needed.  
    2. Follows Florida Statutes and Florida Administrative Codes, 65E-15, Case Management Regional Operating Policy and Florida Medicaid Manual.
    3. Use appropriate crisis intervention skills to anticipate and prevent crises where possible.
    4. Provide services in a way that utilizes the principles of recovery, and treat every client as a partner in a process to better their lives.
    5. Complete treatment plans/reviews, progress notes and other documentation within agency guidelines.
    1. Provide management oversight during hours of operation including evenings and on call as needed.
    1. Monitor revenue and expense reports of programs, and institutes corrective action to ensure compliance with budgetary goals.
    2. Monitor the productivity of the programs and individual staff on an ongoing basis.  Makes recommendations and implements processes to improve productivity as needed. Regularly utilizes reports in the EMR to assess program and staff effectiveness.

      Minimum Qualifications: 
      Education and Experience:   
      A Bachelor’s degree from an accredited university or college, and 2 years of related case management experience to either the adult or child population. 
      Additional Requirements:
      Must be 21 years of age.
      Negative Drug Screening prior to hire and throughout employment.
      Clear Criminal Background Check throughout employment with Suncoast Center and local checks prior to hire and annually along with DCF Level II prior to hire and as required by DCF, and Must have valid Florida Driver’s License Motor Vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance. (must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character).
       
      Working Conditions:  Provide services both in-agency and in the community as required. Schedule may be irregular as emergencies arise.
       
      Computer Literate:   Must have experience working in an electronic medical record.
