Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager.     We are seeking a dedicated and compassionate individual to join our team as an Intensive Case Management (ICM) Case Manager

    Responsibilities:

    • Determine client eligibility for Intensive Case Management (ICM) programming and conduct intake assessments.
    • Review program expectations, outcomes, and confidentiality with each potential client.
    • Develop individualized SMART Self-sufficiency plans for each client enrolled in the ICM program.
    • Evaluate client progress weekly, monthly, or quarterly during active program enrollment to determine the client’s progress toward goals.
    • Work with the client to identify new needs and actions needed.
    • Assist clients in navigating healthcare and social service systems. This may include helping clients apply for benefits and attending and scheduling appointments.
    • Conduct community outreach and maintain a strong network of service providers that meet client needs.
    • The ICM case manager may also be expected to maintain strong relationships with local and state-level refugee and health coordinators to ensure client needs are met.
    • Provide clients with extended orientation on a variety of topics, such as gender-based violence, LGBTQ issues, women’s health, navigating transportation systems, etc.
    • Respond to client crises, with guidance from team leads as needed.
    • Assist clients with requesting financial assistance when applicable and track the received financial aid in case files.
    • Maintain up-to-date documentation in the client’s case file to ensure services are properly documented throughout the client’s service period.
    • Attend relevant training and meetings at the local and national levels.
    • Other duties as assigned.

    Qualifications:

    • BS/BA degree or equivalent work experience
    • A minimum of 2 years of relevant work experience
    • Ability to motivate others & a problem solver. Team player who is willing to assist colleagues
    • Highly motivated self-starter with the ability to meet aggressive deadlines, coordinate multiple projects and tasks simultaneously, and enjoy a fast-paced work environment
    • Excellent time management skills
    • Strong understanding and/or experience working with homeless, low-income, refugees, and recovering populations
    • Possess a high level of emotional intelligence, understanding, and empathy for individuals exhibiting signs of stress.
    • Ability to exercise mature judgment
    • Understanding of and sensitivity to various cultures and religious issues
    • Computer/Mobile devices literacy required: must be proficient with Microsoft Office and Google products and willing to learn other applications as needed
    • Excellent written and verbal communication, interpersonal, administration, and organizational skills.
    • Car, valid driver’s license, good driving record, and proof of auto insurance required
    • Ability to travel to meet with applicants and to meetings/conferences.
    • Some evenings and/or weekends as needed.
    • Bilingual preferred.

    Pay: $21.00 - $23.00 per hour
