Provides hospital discharge planning and social work case management for patients/clients and their families/support systems. Acts as a patient advocate and assists patients/clients to facilitate follow-up health care services and community resources as needed. Coordinates and oversees services of the swing bed program.

Assists hospitalized patients with discharge planning needs by assessing for and anticipating possible post hospital discharge social and financial needs and coordinates with appropriate post discharge support services.
- Promotes and participates in a collaborative approach and serves as a primary point of contact for the interdisciplinary patient care team and other support services to identify problems, formulate treatment plans, and evaluate patient progress.

Assists the patient and their family/support system to arrange for appropriate agency and service referrals to prepare for discharge. This includes, but is not limited to:
- Care Facilities (Skilled Care, Long Term Care, Assisted Living)

Social Assistance Opportunities (Medicaid, SSI, Community Resources)

Demonstrates the ability to work with diverse populations with various knowledge of caring for patients in various developmental stages throughout the life cycle.

Identifies and understands the social and emotional factors underlying patients/client illnesses.
- Evaluates and completes patient assessments to identify potential barriers related to health equity and social determinants of health.

Assist patients and families/support systems in completing necessary economic and healthcare related applications.

Assists patients with advanced care planning and assists with completion of advance directives.

Responsible for assisting with regulatory reporting with other agencies including, but not limited to:
- Law Enforcement

Maintain departmental policies and procedures in accordance with current regulations and critical access hospital conditions of participation.

Conduct educational presentations to audiences such as:
- Internal Staff Inservice Education

Community Education

Collaborates with other departments on resource referral as needed.

Acts as a patient advocate for patient concerns and as a member of the grievance committee.

Reviews program referrals and coordinates admission to swing bed when indicated. This includes coordination with members of the interdisciplinary care team to determine appropriateness for swing bed level of care.

Maintain departmental policies and procedures in accordance with current regulations and critical access hospital conditions of participation.

Reviews records to assure completeness of documentation. Coordinates completion of necessary forms and documentation, including a comprehensive assessment, social assessment, care plans and recapitulation of stays as indicated.

Assists with program marketing and community outreach.
Hours: Minimum 40 hours/week, Monday-Friday, 7:30am-4:00pm
Experience and Skills:
Education, Experience:

Minimum, Bachelors Degree of Social Work or higher education degree.
- 1 year of healthcare setting experience preferred.
Certificates, Licensure, Registrations: All certificates should be obtained within one year of hire, or as approved by manager.
