Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager.     CASE MANAGER (RN) - CASE MANAGEMENT

    Northwest Specialty Hospital is seeking a compassionate, patient-focused RN to join the team!

     

    This position will be two 10-hour shifts! 

     

    This position is responsible for performing individual assessments and care planning, providing clinical education and guidance to patients and their family members, monitoring length of stay throughout hospitalization, and partners with providers for discharge planning. The Case Manager will develop and monitor program compliance with federal and state laws, The Joint Commission, and hospital policies as appropriate. This position requires a high degree of flexibility and adaptability, independent judgement, and confidentiality.

     

    Qualifications and Preferred Experience:

     

    • Previous nursing experience strongly preferred
    • Current RN licensure in the State of Idaho
    • BSN strongly preferred
    • Proof of Basic Life Support (BLS) Certification
    • ACLS certification within six months of employment
    • PALS certification within six months of employment
    • Demonstrated skills in verbal and written English communications for safe and effective patient care and to meet documentation standards
    • Ability to relate and work effectively with others
    • Aware of professional nursing standards and performs in accordance with them
    • 1 – 3 years of previous case management experience is required
