Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. What does this position look like~ A Case Manager is responsible for developing and implementing individual recovery plans. Provides interventions that decrease barriers to service. Collaborates with other treatment providers. Documents observations and information regarding services delivered in progress notes. Assists in the development of social skills and daily living skills. Promotes consumer usage of and facilitates direct access to needed community services. Promotes independence in travel, fiscal management, health care, etc. Provides assertive community outreach. Participates in team meetings and case reviews. Maintain a caseload of ICMS clients.
