Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. We are looking for a Targeted Case Management Manager to play a key role in furthering our mission in our Case Management area.
Summary/Objective:
Reporting to the Senior Director of Disability Services, the Targeted Case Management Manager will provide oversight and support of case managers, ensure advocacy and service delivery to persons with intellectual and developmental disabilities.
Education/Experience:
Bachelor’s Degree in business, Social Work, Human Services, Special Education or other related field. Supervisory experience required.
