    Summary: Under limited guidance, works in conjunction with providers, Coding Professionals, Utilization Review, Compliance, Health Information Management (HIM) and other health team members to provide overall quality and completeness of clinical documentation to reflect clinical treatment, decisions, and diagnosis for both inpatients.

    Responsibilities:

    Representative Duties:
    • Facilitates appropriate clinical documentation to support appropriate diagnosis coding and to ensure the level of service rendered is captured in the medical record.
    • Collaborate with providers to improve and standardize documentation of all conditions, treatments and care plans to accurately reflect the condition of the patient and promote patient care.
    • Documentation should reflect associated MS-DRG assignment, case mix index, severity of illness, risk of mortality, provider/hospital profiling and reimbursement rules.
    • Conduct concurrent medical chart reviews by utilizing hospitals CDI software to identify opportunities and prioritization of chart review. Works with providers to clarify documentation in the medical record.
    • Formulate credible clinical documentation clarifications to improve clinical documentation of principal diagnosis, co-morbidities, present on admission (POA), quality core measures, and patient safety indicators (PSI). Collaborates with HIM coding professional staff to promote complete and accurate clinical documentation and correct negative trends.
    • Acts as a liaison for hospital operations, providers, case managers, coders and other members of the care team to facilitate comprehensive medical record documentation to reflect treatment, decision-making and medical documentation.
    • Participates in ongoing education of providers and staff.
    • Develops educational material and tools relative to documentation improvement practices for providers and staff.
    • Reviews clinical documentation with the coding staff to assign a working DRG.
    • Participate on the CDI Steering Committee as needed, and co-lead (with Coding Professional Leadership) the CDI Task Force.
    • Assumes accountability for achieving program objectives and benchmarks.
    • Collaborates with the leadership to trend analyses and report findings to the CDI Steering Committee and/or reports patient safety documentation concerns within the online incident accident event reporting system.
    • Ensure compliant query/clarification practice. Maintains integrity of data collection.
    • Participates in post-discharge query/clarification reconciliation, collaborating with Coding Professionals.
    • Resolve CDI-Coder DRG discrepancies working in collaboration with the CDI Provider Champion.
    • May evaluate clinical denials to provide clinical insight as a revenue recapture effort.
    • Collaborate with leadership to educate other CDI staff and coding professionals on identified opportunities for improvement.
    • Ensure involvement in quality improvement and patient safety efforts that support the overall mission and vision of the organization.
    • Reviews and explains program metrics, goals and targets and collaborates with leadership on means to meet or exceed those targets.
    • Collects and analyzes data showing the activities performed, results of interactions, improvements made in clinical documentation, and distribution of DRG’s and case mix index.
    • Partners with physician leadership and services lines to compile and distribution information.
    • Maintains current knowledge of trends, regulations and technology related to documentation, coding, reimbursement and regulatory guidelines through professional journals, informal networks, internet sites and local, state, and national professional associations.
    • Performs other duties as assigned.

    • Knowledge of clinical application, medical terminology and abbreviations; anatomy and physiology; major disease process and pharmacology.
    • Knowledge of classification systems CPT, ICD, and HCPCS nomenclature, coding rules and guidelines.
    • Knowledge with Hospital Acquired Conditions (HAC) and Patient Safety Indicators (PSI).
    • Knowledge of coding conventions and rules established by the American Medical Association (AMA), the Center for Medicare and Medicaid (CMS), AHIMA, ACDIS, and other nationally recognized organizations for assignment of diagnostic and procedural codes.
    • Knowledge of Alaska Tribal Health System, ANTHC, and Alaska Native culture.
    • Knowledge of the Privacy Act of 1974 and HIPAA Privacy Rule Act of 1996.
    • Knowledge of CDI and coding software.
    • Knowledge of auditing principles and standards.
    • Knowledge of DRG payer issues and documentation.
    • Skill in operating a personal computer utilizing a variety of applications.
    • Skill in developing and conducting training/educational sessions for diverse audiences.
    • Skill in reading and auditing medical records, utilizing the electronic health record, and identifying documentation improvement opportunities.
    • Skill in understanding medical billing procedures and protocols.
    • Skill in receiving, disseminating information, effectively and appropriately; using analytical and research skills to define and solve problems.
    • Skill in interpreting and applying ethical coding compliance, standards, federal and state laws and regulations; rules; policies and procedures; appropriate codes to diagnosis, procedures, evaluation and management, and supplies.
    • Skill in oral, written communication, and writing technical reports and other materials for presentation.
    • Skill in managing multiple priorities and tasks concurrently and meeting deadlines.
    • Skill in establishing and maintaining cooperative working relationships with others.
    • Ability to proficiently work with the electronic health records and abstract and enter data into all databases used for clinical documentation.

    MINIMUM EDUCATION QUALIFICATION
    An Associate’s degree in Nursing or Health Information Management

    MINIMUM EXPERIENCE QUALIFICATION
    Non-supervisory – Three (3) years of CDI or inpatient coding experience in an inpatient environment. An equivalent combination of relevant education and/or training may be substituted for experience.
    TO INCLUDE
    Supervisory - One (1) year involving employee supervision.

    MINIMUM CERTIFICATION QUALIFICATION
    Must possess one or more of the following: current registered nurse license in the State of Alaska, or certified coding DRG specialist, Registered Health Information Administrator (RHIA), or Registered Health Information Technician (RHIT).

    PREFERRED EDUCATION QUALIFICATION
    A Bachelor’s degree in Nursing or Health Information Management

    PREFERRED CERTIFICATION QUALIFICATION
    Certified Documentation Improvement Practitioner (CDI-P), Certified Coding Specialist Physician-based (CCS-P) preferred, or Certified Clinical Documentation Specialists (CCDS).

    ADDITIONAL REQUIREMENTS
    May be required to work outside the traditional work schedule.
    May be called out to work off-shift in emergency situations.
    According to the needs of the organization, some incumbents in this job class may be required to obtain specific technical certifications and programming language skills.
