Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. The Care Coordinator job responsibilities involve service linkage and care coordination, engaging and working with children, youth, and families with significant behavioral health needs. Care Coordination team members should have a thorough understanding of local communities, be skilled at developing working relationships with community agencies, and can identify potential community supports for development to assist families/caregivers working collaboratively with Child and Family Teams. Care Coordination staff ensure children, youth and families have a voice and choice in all coordinated care and services provided.
