EHR Project Manager, Sr. is responsible for the coordination and completion of complex projects involving the planning, design, and implementation of EHR (electronic health record) software applications. Oversees all aspects of projects. Being an EHR Project Manager, Sr. sets deadlines, assigns responsibilities, and monitors and summarizes progress of project. Builds and maintains working relationships with team members, vendors, and other departments involved in the projects. Additionally, EHR Project Manager, Sr. resolves any development issues or team member performance issues. Prepares project status reports. Requires Epic, Cerner, or similar product certification. Typically requires a bachelor's degree. Requires EPIC Certification. Typically reports to a manager or head of a unit/department. The EHR Project Manager, Sr. manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be an EHR Project Manager, Sr. typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required.     Job Description

    Medical case managers are collaborative partners with each enrolled client. This partnership entails the assessment of client needs, linking with resources, and periodic reassessment with the goal of increased access to and participation in medical care. The roles and responsibilities of the medical case manager are:

    • Maintain a caseload of 45-50 clients living with HIV
    • Provide timely, courteous, and culturally sensitive medical case management services.
    • Be knowledgeable of and adhere to policies and procedures of the Missouri HIV Medical Case Management system as a whole
    • Complete annual and 6-month check-ins in accordance with the Missouri HIV Medical Case Management manual
    • Develop a comprehensive, individualized service plan and ongoing client monitoring to assess the efficacy of the plan
    • Monitor client participation in medical care as defined by the Missouri HIV Medical Case Management Manual
    • Collaborate with the clinical care and prevention teams to promote engagement and retention in medical care
    • Assess high risk behaviors and if indicated develop risk reductions plans with client
    • Assess medication adherence and if indicated, create plans that assists client in becoming medication compliant
    • Coordinate services and assist clients in accessing available entitlements and community programs
    • Document required information and activities in the electronic client database in accordance with the Missouri HIV Medical Case Management manual
    • Follow federal and state statutes regarding confidentiality of client information and records
    • Participate in all required meetings
    • Participate in outreach efforts to provided counseling and education to at-risk populations.
    • Working independently and have excellent organizational skills
    • All other duties as assigned

    Qualifications:

    Educations: RN Degree, BSW or a closely related field. RN/MSW preferred.

    Experience: One year of progressive case management experience with knowledge of the community. Direct experience in HIV/AIDS issues or education is preferred.

    Required Skills/Abilities:

    • Physical, sensory, and cognitive abilities sufficient to perform essential functions..
    • Good interpersonal and written and verbal communication skills
    • Valid driver’s license and own transportation to make home visits. Must be able to transport clients using APO provided vehicle.
    • Must be able to respond to crisis situations with empathy, clarity, calmness, creativity and self-control

    Physical Requirements:

    • Prolonged periods of sitting at a desk and working on a computer and standing, bending and moving.
    • Must be able to lift up to 15 pounds at times.
    • The ability to complete assignments in all types of weather and ever-changing and sometimes allergenic environments in patient homes, including extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc.
    • Some travel required
    • Operates office equipment and manages supplies
    • Computer Skills required
    • Visual acuity to prepare and read reports
    • Hours of duty are normal office hours with occasional evening and weekend tasks.

    Reports to: CM Supervisor or Director of Case Management. In the event that the CM Supervisor and Director of Case Management are unavailable, report to the Executive Director.

    Job Type: Full-time

    Pay: $18.50 - $20.20 per hour

    Expected hours: 37 per week
